Transforming Data into Actionable Knowledge: Methods and Applications to Support Population Health
As a global health community, we are facing the perfect storm of an aging population combined with unsustainable costs of care. Meanwhile, our aging population is presenting the moral and fiscal dilemmas of managing patients longer, with greater chronic diseases. How we prudently deliver better care across the patient continuum — from early to elderly – is reliant on our access to unparalleled patient data inviting new sources of data to support clinical decision making beyond what is available through traditional electronic medical records. This presentation shall introduce strategies for combining clinical and consumer data to top cost populations, developmentally staged upon a comprehensive population health strategy for health systems around the world.
